We are a 70 bed skilled nursing facility located on State Road 16 in Newton County located right outside the sleepy little town of Brook.
We are located on the historic grounds of George Ade's Hazelden Manor.
Ade built an impressive English Manor/Tudor-style home called Hazelden. Ade's home soon became known as the amusement center for the United States, hosting a campaign stop in 1908 by William Howard Taft, a rally for Theodore Roosevelt's Bull Moose Party in 1912, and a homecoming for soldiers and sailors in 1919.
Our historic site is also home to the Hazelden Country Club Golf was first played at Hazelden in 1910. The scenic Iroquois River runs through their historic, par 70, eighteen-hole golf course. Gentle on the beginner yet still challenging for the proficient golfer, Hazelden Country Club is enjoyable for all skill levels.
